Stomach cramps and spotting between periods

Spotting is usually lighter than a period, but its appearance varies. Cramping may occur when the uterus contracts, or when inflammation affects the cervix or pelvic organs. Chlamydia can present with pelvic discomfort and bleeding between periods. Cervical dysplasia, cervical cancer, womb cancer, endometrial polyps, and uterine fibroids are less common possibilities that a clinician may consider, particularly when bleeding is persistent, new, or occurs after sex.

Record the date, amount, colour, pain pattern, sexual activity, contraception, discharge, and any missed periods. A clinician may use this history alongside an examination, pregnancy test, infection testing, or imaging to identify the source.

Use a liner or pad to observe the bleeding rather than relying on memory. Avoid vaginal products that irritate the area, and do not self-treat a possible infection with leftover medicine. Book an appointment if spotting happens repeatedly, lasts beyond a short episode, follows sex, or comes with discharge or pelvic pain. Mention any pregnancy possibility and whether you have reached menopause.

Get urgent care for severe or one-sided abdominal pain, fainting, heavy bleeding, fever, vomiting, or weakness. If pregnancy is possible, pain with bleeding needs prompt assessment. Bleeding after menopause should be assessed even when it is light.

Ask whether you need tests for pregnancy or sexually transmitted infections, a cervical examination, or an ultrasound. Ask what changes should trigger faster review and when to expect results.
