Stiff neck and strep throat

Strep throat is a bacterial infection of the throat. People with it may have sudden throat pain, pain when swallowing, fever, tender swollen glands in the neck and sometimes headache. The glands are not the same as the neck muscles, so gland tenderness can feel different from difficulty turning the head. Tonsillitis can produce a similar picture, while cough and a runny nose may suggest another cause. Strep throat does not explain every stiff neck: severe rigidity, a severe headache, confusion or pronounced illness needs urgent evaluation for a serious complication or a different condition.

Arrange a GP consultation if throat pain is significant, swallowing is difficult, fever persists or swollen glands are tender. A clinician can examine the throat and decide whether testing is useful. Drink regularly, rest and select soft foods. Do not share cups or utensils while an infectious throat illness is possible, and wash hands often. If antibiotic treatment is prescribed, take it exactly as directed and ask what to do if symptoms do not improve or side effects appear.

Seek emergency help for difficulty breathing, inability to swallow saliva, drooling, a muffled voice, severe one-sided throat swelling, confusion, a seizure or a stiff neck with severe headache and fever. Prompt assessment is also needed if you cannot keep fluids down, symptoms worsen quickly or you become unusually drowsy. Do not wait for a routine appointment when the neck becomes rigid and you feel acutely unwell.

Ask whether your symptoms fit strep throat or tonsillitis, whether a throat test is needed, and whether antibiotics would help. Explain when the stiffness began, whether it is muscular or gland-related, and report fever, headache, rash, cough, swallowing difficulty or breathing changes.
