Stiff neck and throat pain: what should you know?

A common cold, influenza or COVID-19 may cause sore throat with tiredness, cough, congestion or fever. Tonsillitis and strep throat can cause painful swallowing, swollen tonsils and tender glands in the neck. Glandular fever may bring pronounced fatigue and swollen glands. Allergic rhinitis can cause throat irritation from drainage, while acid reflux may cause burning, hoarseness or a lump sensation. Neck stiffness may come from tender glands, muscle guarding or a separate strain. The symptom pattern, examination and course help distinguish these possibilities.

Rest and drink fluids in small, frequent amounts. Warm drinks, cool soft foods and salt-water gargling may soothe the throat if swallowing is safe. Avoid smoking and other irritants. Follow label directions for suitable pain or fever relief, and ask a pharmacist if you have a long-term condition, are pregnant or take regular medicines. Check whether the neck moves comfortably and note fever, rash, voice change, cough, exposure to illness and any trouble swallowing.

Seek emergency help for difficulty breathing, inability to swallow saliva, drooling, a muffled voice, rapidly increasing neck or throat swelling, severe one-sided throat pain, confusion or inability to move the neck because of severe stiffness. Obtain prompt medical advice for high fever, dehydration, a spreading rash, worsening symptoms or throat pain that is not settling. A clinician may need to inspect the throat and neck and decide whether testing or treatment is appropriate.
