Stiff neck and tension headache: what should you know?

Short answer
A stiff neck can accompany a tension headache when neck and scalp muscles tighten, but sudden or unusual symptoms need medical assessment.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Common triggers
- Stress, screen posture, jaw clenching, poor sleep and muscle strain
- Try
- Regular breaks, gentle movement, warmth and steady hydration
- Emergency signs
- Sudden severe pain, neurological change, confusion or seizure
What the pattern can suggest
Tension headache is often described as pressure or a tight band rather than a throbbing pain. Neck and shoulder muscle tension may build with stress, poor posture, prolonged screen work, jaw clenching or an uncomfortable sleeping position. The discomfort can spread from the neck towards the back of the head. A headache that is new, rapidly worsening, triggered by exertion, or different from your usual pattern should not simply be labelled tension headache. A clinician considers the headache pattern, neurological symptoms, medicines, blood pressure and other health factors.
Measures to try
Take a break from close screen work, adjust the screen so your head stays level, and relax your shoulders. Gentle neck turns and a warm pack may help if they remain comfortable. Drink fluids, eat regularly and aim for a consistent sleep routine. Keep a headache diary with triggers, duration, location, associated symptoms and medicine use. Frequent pain medicine can itself contribute to recurring headaches, so discuss repeated use with a clinician or pharmacist.
When to seek help
Call emergency services for a sudden severe headache, weakness, facial droop, trouble speaking, fainting, seizure, confusion, vision loss or repeated vomiting. Get urgent advice for fever with marked neck stiffness, a new rash, strong light sensitivity or a headache after a head injury. Arrange a GP Consultation if headaches recur, change pattern, disturb sleep, or do not improve with simple measures.
Questions to ask
Does my description fit a tension headache? Could posture, jaw clenching or medicine use be contributing? Which symptoms should prompt emergency help? Would a headache diary or examination help guide treatment?
