Stiff neck and right shoulder pain: what to know

Short answer
Stiff neck and right shoulder pain can arise from muscles, neck nerves, or shoulder tissues, while the right-sided location alone does not identify the cause.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

What one-sided pain can show
Pain that increases when you turn your neck may involve the cervical spine or its surrounding muscles. Cervical radiculopathy can cause pain from the neck into one shoulder or arm, with altered sensation or weakness. Pain triggered by reaching, dressing, or lifting may be centred in the shoulder; rotator cuff tendinitis, shoulder impingement, calcific tendinitis, or frozen shoulder can produce different movement patterns. After a fall, a rotator cuff tear or other injury needs consideration. A clinician uses the history and examination to separate these possibilities.
What to try
Keep the right shoulder and neck gently mobile within a comfortable range. Avoid repeatedly testing painful movements, carrying heavy loads on that side, or working overhead during a flare. Raise your screen, support the forearm, and alternate tasks. Use a wrapped cold pack after a recent strain or warmth for muscle tightness, choosing whichever feels more comfortable. Ask a pharmacist about pain medicine that fits your health history. If symptoms persist, an examination can compare both sides and check strength, sensation, and range of motion.
When to seek care
Call emergency services for chest pressure, breathlessness, fainting, sweating, or pain spreading to the jaw or arm, even if the discomfort is on the right. Seek urgent assessment after significant trauma, visible deformity, severe swelling, or inability to raise the arm. New weakness, marked numbness, balance trouble, fever with severe neck stiffness, or a severe unusual headache are also warning signs. Book a review when the problem limits sleep, work, or daily tasks or does not improve.
Questions for your doctor
Which movement best identifies whether my neck or shoulder is involved? What should I change at work or during exercise? Do my arm symptoms suggest nerve irritation? What is a suitable plan if movement remains restricted?

Questions people ask
Does right-sided pain mean the shoulder is the problem?
No. Neck muscles, cervical joints, and nerves can refer pain into the right shoulder, and shoulder tissues can also cause neck tension.
Should I keep using the painful arm?
Gentle movement is useful, but reduce heavy or overhead activity and stop if pain becomes sharp or weakness appears.
