Where the pain may begin

Neck movement that reproduces shoulder pain can point toward cervical muscles, joints, or cervical radiculopathy, particularly when tingling or weakness reaches the arm. Pain mainly triggered by lifting, reaching, or lying on the shoulder may involve rotator cuff tendinitis, shoulder impingement, or calcific tendinitis. Gradually increasing restriction in nearly every shoulder direction can occur with frozen shoulder. A fall or sudden force raises concern for a rotator cuff tear or another injury. These are patterns for discussion, not a diagnosis from symptoms alone.

Practical care

Keep both the neck and shoulder moving gently, stopping before sharp pain. Reduce heavy lifting and repetitive overhead activity for now, while avoiding complete shoulder immobilisation unless advised. Set screens at eye level, support the arm when seated, and change position often. Warmth may relax tight muscles; a wrapped cold pack may soothe a recent flare. A clinician or physiotherapist can compare neck and shoulder movement, strength, and sensation to identify the main source and plan exercises.

When to seek care

Get urgent help for chest pressure, breathlessness, fainting, sweating, or pain spreading into the jaw or arm. Seek prompt assessment after a fall or impact if the shoulder looks deformed, swelling is marked, or you cannot use the arm. New arm weakness, significant numbness, fever with severe neck stiffness, or a severe unusual headache also needs urgent care. Arrange a non-urgent review if movement remains limited, pain disrupts sleep, or symptoms continue despite simple measures.
