What should you know about statin rhabdomyolysis onset?

Symptoms may appear after a statin is started, after its dose changes, or when another medicine changes how the body handles it. They can also emerge during dehydration, infection, or heavy physical exertion. Timing alone cannot establish the cause, because ordinary exercise soreness and several other conditions can feel similar.

Rhabdomyolysis involves muscle-cell injury and release of muscle contents into the blood. Widespread tenderness, unusual weakness, difficulty climbing stairs, brown or tea-coloured urine, and a drop in urine output are more concerning than mild, localised soreness.

Report new or escalating muscle symptoms to the prescribing clinician without delay. Mention when they began, where they are located, recent exercise, fever or vomiting, and any new prescription, over-the-counter medicine, or supplement. A clinician may arrange blood tests for muscle injury and kidney function and give specific instructions about the next statin dose.

Avoid strenuous exercise until you have been assessed. Follow individual fluid advice, particularly if you have heart or kidney conditions that limit drinking.

Go to urgent care if weakness is severe, pain is rapidly worsening, urine becomes dark, urination decreases, or you feel faint, confused, or seriously unwell. Do not wait for a routine appointment when these warning signs are present.

Ask how soon your symptoms should be tested, which medicines may interact with your statin, and what to do if symptoms return after treatment is adjusted. Clarify when gentle activity can resume and how your cholesterol plan will be followed.
