What should you know about stages of Asherman syndrome?

Short answer

Asherman syndrome is scar tissue inside the womb; descriptions of mild, moderate or severe disease reflect how much of the cavity is affected and whether fertility or bleeding is impaired.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

How severity is considered

Asherman syndrome, also called intrauterine adhesions, occurs when bands of scar tissue join parts of the womb cavity. A person may have very light or absent periods, pelvic pain around an expected period, difficulty becoming pregnant or repeated pregnancy loss. Symptoms do not always show how extensive the adhesions are.

Clinicians may describe adhesions by their location, density and the amount of cavity involved rather than using one universal staging system. The usual aim is to see the cavity clearly and understand how the lining may respond.

Assessment and treatment

Arrange a gynaecology assessment if periods change after a uterine procedure, pregnancy loss or infection, or if fertility is affected. Operative Hysteroscopy can inspect the cavity and may remove adhesions. Further care can include follow-up imaging, hormonal support or fertility planning, depending on findings.

Tell the clinician about previous Surgical Management of Miscarriage, caesarean birth, retained pregnancy tissue or uterine infection. Bring details of cycle changes and any prior scans or procedures.

When to seek care

Seek prompt care for severe pelvic pain, fever, foul-smelling discharge, heavy bleeding or feeling faint, especially after a procedure. Arrange a non-urgent review for Periods Stopped, markedly lighter bleeding, ongoing cyclical pain or difficulty conceiving. These symptoms can have several causes, so examination is needed rather than self-diagnosis.

Questions for your doctor

How were the adhesions identified and how extensive are they? Could they explain my bleeding or fertility concerns? Is hysteroscopy recommended, and what follow-up will check the cavity? What should I do if pain, fever or bleeding occurs after treatment?

Questions people ask

Does a stage label predict fertility on its own?

No. Fertility also depends on the cavity, the uterine lining, ovulation, sperm factors and other medical details.

Can Asherman syndrome return after treatment?

Adhesions can recur in some people, which is why follow-up is planned according to the original findings and treatment.
