Stage four ovarian cancer: what to know

Staging describes where ovarian cancer is found, not how one person will feel or how treatment will work. Doctors review scans, examination findings, and tissue results to understand the spread. The cancer may involve the abdominal lining, fluid around the lungs, or another distant site. The exact pattern matters because it helps the team choose treatment and decide whether surgery is useful.

Testing may include imaging such as a pelvic ultrasound and laboratory or tissue assessment. Genomic tumour profiling can identify tumour features that may guide treatment choices. BRCA genetic testing may also be discussed because inherited findings can affect care planning and family conversations.

Treatment is individualised. Chemotherapy may be used to slow or shrink cancer throughout the body. Surgery, including ovarian cancer debulking surgery or oophorectomy, may be considered when removing visible disease is feasible and appropriate. Your oncology team weighs the likely benefit, recovery, other health conditions, and your priorities.

Palliative care can be introduced at any stage alongside cancer treatment. It focuses on pain, nausea, appetite, breathlessness, sleep, emotional strain, and practical support. Keep a record of symptoms, medicines, and changes in eating or bowel function so appointments can address them directly.

Contact your oncology service promptly for worsening abdominal swelling or pain, repeated vomiting, inability to keep fluids down, new breathlessness, fever, confusion, or very little urine. Seek emergency help for severe breathing difficulty, fainting, sudden severe pain, or heavy bleeding. These symptoms can have several causes, but they need timely assessment.

Ask where the cancer has spread, what the treatment aims are, whether genomic or BRCA testing is suitable, and how each option may affect daily activities. Ask who to call outside clinic hours and which symptoms should trigger same-day advice. You can also ask about palliative care, nutrition, fertility or menopause concerns, and support for family members.
