What should you know about stage four metastatic prostate cancer?

Metastatic prostate cancer means prostate cancer cells are present outside the prostate, commonly in nearby or distant lymph nodes, bones, or organs. The location and extent of spread help the team choose treatment and plan monitoring. Bone involvement may cause pain, reduced movement, or a weakened area; it can also affect the spine and nerves. Difficulty passing urine may come from prostate enlargement, treatment effects, infection, or cancer-related narrowing, so the symptom needs assessment rather than a diagnosis based on the symptom alone.

Stage describes where the cancer is, not a complete forecast for one person. The pathology, prostate-specific antigen pattern, previous treatments, other health conditions, symptoms, and how the cancer responds over time all add context.

Your oncologist may discuss hormone-based treatment, medicines that act throughout the body, radiotherapy for a painful or threatening area, or targeted radioisotope therapy for selected disease patterns. Bone-strengthening therapy may help protect bone health when cancer or treatment affects the skeleton. Surgery such as prostate removal is used in selected situations and is not suitable for every metastatic presentation; robotic cancer surgery describes an approach, not a guarantee of a particular result. Ask how each option fits your disease, previous treatment, daily life, and priorities. Keep a current list of medicines and report new pain, urinary changes, weakness, or side effects.

Seek emergency help for new loss of bladder or bowel control, inability to pass urine, sudden leg weakness or numbness, severe back or neck pain, or difficulty walking. These can signal pressure on the spinal cord or another urgent problem. Contact the cancer team promptly for fever, uncontrolled pain, blood in the urine, worsening urinary difficulty, a fall, or new confusion. Do not wait for a routine review if a symptom is rapidly worsening; early assessment can identify urinary blockage, infection, fracture risk, treatment toxicity, or another cause.

Do I need an assessment of my bones or spine? Which urinary symptoms need same-day advice? Would radiotherapy, bone-strengthening therapy, or palliative care help alongside cancer-directed treatment?
