Stage 4 pancreatic cancer: what to know

Pancreatic cancer can spread to nearby tissues, the abdominal lining, the liver, or other distant sites. The spread may affect digestion, bile flow, appetite, energy, and pain. Yellowing of the skin or eyes, dark urine, pale stools, itching, abdominal pain, and weight change should be reported.

The team considers the cancer’s location and biology, your symptoms, nutrition, organ function, and general health. Stage describes spread; it cannot determine exactly what will happen to one person.

Chemotherapy may slow cancer activity or reduce symptoms. Radiotherapy can be considered for selected painful or troublesome areas. A Whipple procedure is generally used only in carefully selected localised situations, so stage 4 disease often requires a different approach. Tumour or inherited features may make PARP inhibitor therapy relevant for some people.

Ask for help with pancreatic enzyme replacement, nausea, pain, diabetes management, appetite, and meals. Palliative care can work alongside cancer treatment to support comfort, function, emotional wellbeing, and family needs.

Contact the care team promptly for increasing jaundice, fever, persistent vomiting, inability to drink, worsening abdominal swelling, uncontrolled pain, or confusion. Seek emergency help for severe abdominal pain, fainting, vomiting blood, black stools, severe breathlessness, or inability to stay awake.
