Stage 4 metastatic prostate cancer: what to know

Cancer cells may travel to lymph nodes, bones, or other organs. The location and extent of spread, the cancer’s features, previous treatment, and your general health all shape the care plan. Bone involvement can cause pain, weakness, or a higher risk of fracture, while pressure near the bladder or spine can affect urination or movement.

Tests may include scans, blood tests, and review of biopsy or molecular results. Stage 4 describes where the cancer is; it does not by itself predict exactly how one person will respond to treatment.

Ask the oncology team to explain the treatment goal, how progress will be checked, and which symptoms should be reported between visits. Hormone-blocking treatment, medicines aimed at the cancer, radiotherapy, or carefully selected procedures may be considered. Radiotherapy can help with a painful or threatening area, while bone-strengthening therapy may support bones affected by cancer.

Keep a record of pain, urinary changes, new weakness, medicines, and questions. A balanced eating plan, gentle activity suited to your strength, and support for sleep and mood can make treatment easier to manage.

Contact your oncology team promptly for new or worsening bone pain, trouble passing urine, fever during treatment, repeated vomiting, or sudden loss of strength. Seek emergency help for severe back pain with leg weakness or numbness, loss of bladder or bowel control, inability to pass urine, chest pain, or difficulty breathing.
