What should you know about SSRIs and tamoxifen?

Tamoxifen is changed by the body into active forms that help it act on hormone-sensitive breast cancer cells. Some selective serotonin reuptake inhibitors, or SSRIs, can inhibit the enzymes involved in that change. The effect depends on the particular SSRI, dose, other medicines and individual health. This does not mean that an antidepressant must be stopped or that tamoxifen should be avoided. It means the oncology, prescribing and mental-health teams need the same medication list and should weigh emotional wellbeing alongside cancer treatment.

Give your team the names of all prescription medicines, over-the-counter products and supplements, including why each is taken. Ask the prescriber to check the SSRI before it is started or changed. Never stop an SSRI suddenly unless a clinician gives a specific plan, because withdrawal symptoms and a return of depression or anxiety can follow. Do not alter tamoxifen timing or dose on your own. Your team may discuss another antidepressant or another way to manage symptoms, but the choice should reflect your mental health, cancer treatment and other medicines.

Contact your care team promptly if you have missed repeated tamoxifen doses, develop troubling mood changes, or notice new symptoms after a medicine change. Seek urgent help for thoughts of self-harm, severe allergic symptoms, chest pain, sudden breathlessness or unusual heavy bleeding. Keep taking medicines as prescribed while waiting for advice unless emergency clinicians tell you otherwise.
