Spotting with white discharge during pregnancy

Pregnancy often increases thin, milky-white discharge. Light spotting may come from a sensitive cervix, including after sex or an examination. Discharge that becomes yellow, green, grey, foul-smelling, thick and clumpy, or associated with itching or burning may point to infection and needs assessment.

Blood mixed with watery fluid, a gush or ongoing trickle of fluid, pelvic pressure, cramps, or lower abdominal pain should be reported promptly. A clinician can assess the cervix, test for infection, and check the pregnancy according to your symptoms and stage.

Contact your midwife, obstetric clinician, or maternity service and describe the discharge, blood colour, amount, smell, and timing. Use a pad rather than a tampon to observe fluid. Do not douche or use an unprescribed vaginal treatment. Note itching, pain when urinating, cramps, fever, and whether sex preceded the spotting.

Seek emergency assessment for heavy or increasing bleeding, severe or one-sided pain, fainting, major dizziness, fever, shoulder-tip pain, or feeling very unwell. Contact maternity care urgently for watery fluid leakage, unpleasant-smelling discharge, or regular painful cramps.

Ask whether the discharge needs a swab or urine test, whether the bleeding changes your pregnancy monitoring, and which signs require immediate return. Mention your pregnancy stage, recent sex or examination, medicines, and any history of vaginal infections.
