Spotting with Cramps During Pregnancy

Light bleeding may occur when the cervix is sensitive, including after sex or an examination. Cramping can also happen as the uterus changes. Bleeding and pain can sometimes signal a problem such as miscarriage or an ectopic pregnancy, which needs prompt medical assessment.

The colour, amount, timing, and pattern of bleeding help a clinician assess it. Note whether blood is pink, brown, or bright red; whether it is only on wiping or needs a pad; and whether cramps are mild, one-sided, constant, or worsening. Pregnancy-related bleeding cannot be assessed safely from symptoms alone.

Other possibilities include cervical irritation, infection, or a pregnancy-related change that is not dangerous. A clinician may ask about your pregnancy stage, examine you, and arrange an ultrasound or blood tests when appropriate. Do not assume that a small amount of spotting rules out a significant concern.

Contact your obstetrician, midwife, or maternity assessment service and describe the bleeding and cramps. Use a pad rather than a tampon so you can observe the amount, and record when the symptoms started. Until you receive advice, avoid sex if it triggers bleeding or pain and do not place anything in the vagina.

If you know your blood group or have been told you are rhesus negative, mention this when you call because your maternity team may need to consider additional care. Take your pregnancy details and medication list with you if you are assessed.

Seek urgent medical care for heavy or rapidly increasing bleeding, severe or worsening abdominal pain, pain mainly on one side, shoulder-tip pain, dizziness, fainting, marked weakness, fever, or feeling very unwell. These signs can occur with conditions that require immediate assessment.

If you cannot reach your pregnancy care team promptly, use local emergency services. Do not drive yourself if you feel faint or unsafe. Even without warning signs, report any bleeding during pregnancy so a clinician can advise you based on your pregnancy stage and history.

Ask what may be causing the bleeding, whether you need an examination or ultrasound, and which changes should prompt another call. You can also ask whether to pause sex or exercise, how to manage cramps safely, and when follow-up should happen.
