Spotting one day after sex: what to know

Friction can irritate the vaginal or cervical tissue, particularly when there is not enough lubrication. Hormonal contraception, changes around ovulation, or an approaching period can also cause light bleeding. An infection such as Chlamydia may cause bleeding after sex, sometimes with discharge, burning, pelvic discomfort, or no other obvious symptom. Cervical Dysplasia and Cervical Cancer can present with bleeding after sex, so repeated or unexplained episodes deserve assessment. Polyps and fibroids are other possible sources in the reproductive tract.

The amount, colour, timing, recurrence, pain, discharge, pregnancy possibility, and contraception history help a clinician narrow the cause. A single small episode is different from bleeding that returns or becomes heavier.

Note when the spotting started, how long it lasted, and whether it followed penetration. Record any pain, unusual discharge, odour, fever, urinary symptoms, or bleeding at other times. Pause penetrative sex if it brings on bleeding or discomfort, and use gentle care while the tissue settles. Arrange a routine appointment if the spotting recurs, follows sex repeatedly, or has no clear explanation. A clinician may ask about periods and medicines, examine the vagina and cervix, and offer infection testing or other investigations.

Seek urgent medical help for heavy or rapidly increasing bleeding, severe or one-sided pelvic pain, faintness, marked weakness, fever, or shoulder pain. If pregnancy is possible, bleeding with pain, dizziness, or fainting needs prompt assessment because an ectopic pregnancy can be dangerous. Contact a clinician soon for bleeding after sex that keeps returning, occurs after menopause, or comes with persistent pelvic symptoms.
