Spotting the next day after sex: what to know

Blood may be noticed later when a small amount of bleeding from friction, dryness, or a sensitive cervix leaves the vagina after intercourse. The timing can also overlap with a period, ovulation-related changes, infection, pregnancy, polyps, fibroids, or cervical conditions. A delayed appearance does not show which explanation applies. Repeated bleeding after sex is a reason to arrange an examination even when each episode is small.

Use a pad or liner so you can observe the amount and colour, and record when it stops. Avoid sex if it causes pain or brings the bleeding back; lubrication may reduce friction when you resume, provided there is no reason to avoid sex. Consider a pregnancy test when pregnancy is possible and seek sexual-health advice if infection exposure is a concern. Book a review for recurrence, persistent bleeding, pain, discharge, or a change from your usual pattern.

Get urgent care for heavy bleeding, intense or one-sided pelvic pain, faintness, shoulder-tip pain, fever, or a positive pregnancy test with bleeding. Seek prompt advice for bleeding that does not settle, repeated post-sex episodes, or discharge with a strong odour. Do not wait for another episode if symptoms are severe.

Tell the clinician when sex occurred, when bleeding started, whether penetration was painful, and whether you have contraception, pregnancy risk, discharge, or urinary symptoms. Ask whether you need pregnancy, infection, or cervical tests and whether any temporary changes to sexual activity are advised.
