Spotting like a period in early pregnancy

Light staining may be pink, brown or red and may appear briefly. It can follow cervical irritation, including after sex or an examination, but early-pregnancy bleeding can also signal a complication. A period-like appearance cannot distinguish these possibilities. Note whether the blood is only on wiping or continues onto a pad, and whether cramps are mild, worsening or one-sided.

Other clues include clots, tissue, shoulder-tip pain, dizziness, faintness, fever and unusual discharge. A clinician may ask about the pregnancy dates, prior pregnancy history and any fertility treatment. Call your maternity service or doctor promptly and describe the bleeding accurately. Wear a pad rather than using a tampon, record when it began, and keep track of pain and clots. Follow instructions about tests or follow-up. Do not assume that stopping activity will identify the cause, and do not take new medicines without checking their suitability in pregnancy.

Go to urgent care for bleeding that becomes heavy, severe or worsening pain, one-sided abdominal pain, faintness, marked weakness, shoulder-tip pain, fever or feeling acutely unwell. These warning signs need immediate assessment, even if the pregnancy has not yet been confirmed by a scan. If symptoms are mild but bleeding continues or returns, arrange prompt clinical review.

Ask whether the bleeding pattern needs an examination, blood tests or ultrasound; what follow-up is planned; which activities or medicines to avoid; and exactly where to go if the bleeding or pain changes.
