Possible explanations

A small amount of bleeding may occur around ovulation, after starting or missing hormonal contraception, or when contraception is changed. Infection, including chlamydia, can cause bleeding between periods or after sex, especially with pelvic pain, unusual discharge, or pain when urinating. Polyps, fibroids, cervical dysplasia, and less commonly cervical or womb cancer can also cause abnormal bleeding. A symptom does not identify its cause; an examination may be needed.

How to respond

Record the date, colour, amount, relation to sex or your period, contraception, and any pain or discharge. Use a pad if needed and arrange a routine appointment if the pattern continues, returns, or follows sex. A clinician may discuss a pregnancy test, infection testing, cervical screening history, or an examination based on your symptoms and age.

When care is urgent

Get urgent help for heavy bleeding, severe pelvic or abdominal pain, faintness, dizziness, fever, or feeling very unwell. Seek prompt assessment for bleeding after menopause or bleeding with a possible pregnancy. Emergency symptoms are judged by severity and how you feel, not by whether spotting is a common cycle pattern.
