When can spotting occur in pregnancy?

Spotting may appear very early, around the time an embryo attaches to the womb lining. The cervix also becomes more sensitive during pregnancy, so light bleeding can follow sex or an examination. Bleeding can occur at other times as well, including with infection, an early pregnancy loss, or an ectopic pregnancy.

The timing, colour, and amount provide useful context but do not diagnose the reason. Pelvic or abdominal pain, especially on one side, changes the level of concern. An ectopic pregnancy can present with spotting and pain, so symptoms deserve attention even when bleeding is light.

Contact your maternity team or a clinician and explain how far along you may be, when spotting began, and whether pain or dizziness is present. Wear a pad rather than a tampon while monitoring the bleeding. Record changes, clots, activity before the episode, and any discharge or fever.

A clinician may recommend examination, pregnancy testing, blood tests, or ultrasound. A transvaginal ultrasound can help assess the pregnancy location and development when appropriate. Follow-up may be needed if the first assessment is too early to provide a clear answer.

Go to emergency care for heavy bleeding, severe or one-sided abdominal pain, shoulder-tip pain, fainting, collapse, severe dizziness, fever, or a rapid decline in how you feel. Do not wait for bleeding to become heavy if severe pain or faintness is present.
