What should you know about spotting immediately after sex?

Bleeding after sex may come from dryness, friction, a small surface tear, inflammation, or a sensitive cervix. An infection such as chlamydia can cause bleeding and may also cause discharge, pelvic discomfort, or pain during sex. Cervical dysplasia and cervical cancer can present with bleeding after sex, but spotting alone cannot identify either condition. Polyps or other changes in the reproductive tract are also possible.

The pattern matters: note whether bleeding is a one-off event, follows deeper penetration, occurs with a new partner, or appears after menopause or during pregnancy. Those details help determine the right assessment.

Pause sex if it causes pain or more bleeding, and use a pad or liner to observe the amount. Record timing, colour, pain, discharge, contraception, and pregnancy possibility. Arrange a clinician visit if the bleeding returns, occurs without obvious irritation, or comes with discharge or pelvic pain. Testing may include a pregnancy check, infection testing, or an examination of the vagina and cervix.

Seek urgent help for heavy bleeding, severe pelvic or abdominal pain, fainting, marked dizziness, fever, or feeling very unwell. If pregnancy is possible, one-sided pain, shoulder-tip pain, or increasing bleeding needs prompt assessment. Do not wait for another episode when bleeding is persistent or accompanied by significant symptoms.

Ask whether the bleeding pattern suggests irritation, infection, or a cervical cause; whether you need testing; and what changes should trigger urgent care.
