Spotting During Sex While Pregnant: What to Know

Light pink or brown spotting after penetration may occur because pregnancy increases blood flow and makes the cervix easier to irritate. Bleeding can also have other causes, so its timing, amount, colour, and accompanying symptoms matter.

A small amount noticed only when wiping, soon after sex, may settle as the irritated tissue recovers. Bleeding that continues, returns, or occurs without a clear connection to intercourse should be discussed with your maternity team. Possible explanations include cervical irritation, infection such as Chlamydia, or changes affecting the cervix or uterus.

Spotting alone cannot identify the cause or predict how a pregnancy is progressing. Assessment may involve questions about your pregnancy stage, an examination, and checks chosen by your clinician. Do not assume that a small amount is harmless if you have pain, dizziness, fever, fluid leakage, or reduced fetal movement when you normally feel movement.

Stop intercourse for now and use a pad rather than a tampon so you can observe the amount and colour. Note when it began, whether it followed penetration, whether it is increasing, and whether cramps or other symptoms occur. Contact your obstetric or midwifery team for personalised advice, even if the spotting stops.

Avoid putting anything in the vagina until a clinician advises that it is appropriate. If you have been told to follow pelvic-rest precautions, continue them and mention any bleeding at your next contact. Keep your pregnancy records available when seeking care.

Seek urgent medical help if bleeding is heavy or bright red, contains clots, is increasing, or comes with severe or one-sided abdominal pain, shoulder pain, faintness, marked weakness, fever, or fluid leaking from the vagina. Contact emergency services if you feel seriously unwell or might pass out.

Later in pregnancy, bleeding with regular contractions, pelvic pressure, or reduced fetal movement needs urgent assessment. Do not drive yourself if you are faint, in severe pain, or bleeding heavily. A clinician can decide whether you need immediate examination, monitoring, or testing.

Ask what level of bleeding should prompt urgent review, whether you should avoid sex or vaginal examinations for now, and which symptoms to monitor at your stage of pregnancy. You can also ask whether infection testing, an ultrasound, or another check is appropriate based on your symptoms and pregnancy history.
