Understanding early pregnancy spotting

During the first trimester, light staining may follow sex or an examination because pregnancy increases blood flow to the cervix. Infection, cervical irritation, or a small cervical polyp may also cause bleeding. Spotting can occur with a pregnancy that continues normally, but it can also occur with miscarriage or an ectopic pregnancy. The shade of blood does not diagnose the cause. One-sided pain, shoulder-tip pain, dizziness, or faintness is particularly concerning because an ectopic pregnancy can become dangerous quickly.

Write down when the spotting started, whether it is increasing, the colour, clots, and any cramps or pain. Mention the date of your last menstrual period, how far along you may be, recent sex or examination, previous ectopic pregnancy or miscarriage, and any fertility treatment to the clinician.

Steps to take

Call your maternity service, obstetrician, midwife, or early pregnancy unit when spotting begins, even if it is light. Use a pad to monitor the amount and avoid tampons. Ask before taking medicines for pain, particularly if you are unsure what is suitable in pregnancy. You may be offered an examination, ultrasound, blood tests, or repeat assessment; the choice depends on symptoms and how far the pregnancy has progressed. Keep your appointment if spotting stops.

When it is an emergency

Go to emergency care for heavy or rapidly worsening bleeding, severe cramps, one-sided abdominal pain, shoulder-tip pain, faintness, collapse, fever, or feeling very unwell. These symptoms need urgent assessment rather than observation at home. If you cannot reach your pregnancy team and symptoms are severe, use emergency services.
