Spotting After Sex in the First Trimester

Pregnancy increases blood flow and sensitivity around the cervix, so contact during sex can cause light spotting. Irritation, infection, or a small vaginal tear are other possibilities. Spotting does not by itself show whether a pregnancy is progressing normally, and its colour or timing cannot establish the cause.

Bleeding can also occur with pregnancy complications, including miscarriage or ectopic pregnancy. The risk cannot be judged from the amount alone. Tell your maternity team when the bleeding began, whether it followed sex, and whether you have cramps, one-sided pain, shoulder-tip pain, dizziness, or fever.

Contact your midwife, obstetric team, early-pregnancy service, or doctor for advice, particularly if the spotting continues or returns. Use a pad to observe the amount rather than a tampon, and avoid sex until you have received advice if it brings on bleeding or discomfort. Keep details of colour, clots, pain, and any medical conditions or previous pregnancy concerns ready for the call.

Seek emergency care for heavy bleeding, severe or one-sided abdominal pain, shoulder-tip pain, fainting, dizziness, fever, or feeling very unwell. These symptoms can signal a complication that needs immediate assessment. Do not drive yourself if you feel faint.
