Spotting after sex: could I be pregnant?

Spotting after sex may happen in early pregnancy, but it cannot confirm pregnancy and should be checked with appropriate testing. Intercourse can also cause friction-related irritation, dryness, or bleeding from a sensitive cervix. The same symptom can occur when pregnancy is not present.

Think about the date of sex, contraception, your expected period, and whether the spotting is linked only to intercourse. Bleeding between periods can occur with infections such as chlamydia or with cervical, uterine, or hormonal causes. Cervical dysplasia, polyps, fibroids, cervical cancer, and womb cancer may present with abnormal bleeding, but spotting does not tell you which condition, if any, is responsible.

Record the amount, colour, duration, cramps, pain during sex, discharge, and any missed period. This gives a clinician a clearer starting point.

Take a home pregnancy test when its instructions indicate the result should be reliable, usually around the expected period. If the result is negative and the period remains absent, repeat testing as directed or contact a clinician. Seek timely advice about emergency contraception after recent unprotected sex if pregnancy is not wanted. Arrange a routine review if spotting recurs.

Seek urgent medical help for severe or one-sided pelvic pain, shoulder-tip pain, faintness, heavy bleeding, or a positive pregnancy test with pain or bleeding. These symptoms can occur with an ectopic pregnancy or significant bleeding. Fever, chills, or feeling very unwell also require prompt assessment.
