Spotting after sex: what should you know?

Spotting after sex can have a minor cause, but repeated or unexplained bleeding needs medical assessment. Friction, vaginal dryness, or a small surface tear may cause light blood soon after intercourse. Bleeding can also occur with hormonal contraception, an infection, or a change affecting the cervix.

The timing, amount, colour, and whether it happens again help a clinician understand the pattern. Cervical dysplasia, chlamydia, cervical cancer, endometrial polyps, uterine fibroids, and womb cancer can present with bleeding, although spotting alone cannot identify the cause. A breast-style self-check is not relevant here: the source is usually the vagina or cervix, and an examination may be needed.

Keep a note of when the spotting starts, how long it lasts, whether sex was painful, and whether you have discharge, pelvic pain, itching, or bleeding between periods.

Pause intercourse if it brings on bleeding or pain, and avoid inserting products until the bleeding settles. Arrange a routine appointment if this is new, returns, happens after menopause, or follows sex with a new partner. A clinician may discuss pregnancy testing, infection tests, cervical screening history, and an examination. Treatment depends on the finding; a radical hysterectomy is relevant only to selected diagnosed cancers, not ordinary spotting.

Seek urgent help for heavy bleeding, severe or one-sided pelvic pain, faintness, marked weakness, fever, or bleeding during a possible pregnancy. These features can need prompt assessment for causes such as pregnancy-related complications or infection. If bleeding is light but keeps recurring, book an appointment rather than waiting for it to disappear on its own.
