Spotting after your period with cramps

Mild cramping can accompany ovulation, a change in contraception, or the uterus passing a small amount of blood. Chlamydia and other pelvic infections may cause bleeding between periods with pelvic pain, discharge, fever, or pain during sex. Fibroids and endometrial polyps can contribute to irregular bleeding and cramping. Cervical dysplasia or cervical cancer can present with abnormal bleeding, but the combination of spotting and cramps cannot identify the cause.

Describe whether the cramps are central or one-sided, constant or intermittent, and mild or worsening. Track the bleeding colour and amount, bowel or urinary symptoms, discharge, fever, recent sex, contraception, and the possibility of pregnancy. Arrange an appointment if the episode recurs, interferes with daily activities, or does not settle. A clinician may use this history to decide about pregnancy, infection, cervical, or pelvic assessment.

Get urgent help for severe, sudden, or one-sided pelvic pain, heavy bleeding, faintness, dizziness, fever, or vomiting. If pregnancy is possible, do not wait for symptoms to become severe: bleeding with pain or feeling faint needs prompt advice. These warning signs matter even when the initial spotting looked minor.
