Spotting after orgasm during pregnancy

Pregnancy increases blood flow to the cervix, which can make its surface bleed after sexual activity or an orgasm. Mild uterine tightening after orgasm may also be noticed. Spotting can instead relate to irritation, infection, cervical dysplasia, or a problem involving the pregnancy or placenta. The cause cannot be identified from colour or amount alone, and bleeding after orgasm does not prove that a serious problem is present.

Your stage of pregnancy, pain, fluid loss, fetal movement, previous pregnancy history, and examination findings all affect the advice. Some people may be told to avoid sex temporarily while they are assessed.

Stop sexual activity for now and contact your midwife, obstetrician, or maternity assessment service. Record the time, colour, amount, clots, cramps, and whether the spotting followed penetration or orgasm. Use a pad rather than a tampon so you can observe the bleeding. Do not self-treat with vaginal products or medicines unless your pregnancy team recommends them. Follow any advice about examination, ultrasound, infection testing, or temporary pelvic rest.

Seek urgent maternity care for heavy or increasing bleeding, severe or one-sided abdominal pain, regular painful tightenings, fluid leaking, fever, dizziness, faintness, or reduced fetal movement later in pregnancy. Do not wait for a routine appointment when these signs occur.

Ask whether you need an examination, whether sex should be avoided, how to monitor fetal movement, and which changes require immediate attendance. Mention your gestational age and any previous bleeding or pregnancy complications.
