Spotting after non-penetrative sexual activity: what to know

Friction from touching, rubbing, or a sex toy can cause a small surface abrasion, particularly when tissue is dry or sensitive. A vaginal infection or sexually transmitted infection can make tissue inflamed and more likely to bleed; Chlamydia may present with bleeding between periods or after sexual contact. Blood can also come from the cervix or uterus rather than the area that felt irritated.

Cervical dysplasia, cervical cancer, endometrial polyps, uterine fibroids, and womb cancer can present with unexpected bleeding. Spotting after an activity does not establish that any of these is the cause. The pattern, examination, and testing provide the useful information.

Pause the activity if it causes pain or fresh bleeding. Note whether the blood was on tissue, underwear, or a toy, and record its colour, amount, and duration. Avoid further friction until soreness settles; use a plain lubricant in future if dryness may have contributed, and avoid products that sting. Arrange a clinical review when bleeding recurs, happens without obvious friction, follows sex repeatedly, or comes with discharge, odour, burning, or pelvic pain. Testing can check for infection and an examination can identify a cervical or vaginal source.

Get urgent help for heavy or worsening bleeding, severe pelvic or abdominal pain, faintness, marked weakness, fever, or foul-smelling discharge. If pregnancy is possible and bleeding occurs with one-sided pain, shoulder pain, or dizziness, seek urgent assessment for ectopic pregnancy. The amount of blood is not the only factor; severe symptoms matter even when bleeding looks light.

Ask whether the bleeding appears to be from surface irritation, the cervix, or the uterus; whether infection testing is needed; and what changes should prompt a return visit.
