Spotting after a bowel movement in early pregnancy

Constipation, straining, or haemorrhoids can cause blood from the anus, often seen on toilet paper after passing stool. Blood from the vagina may be noticed on underwear or a liner and can occur for several reasons in early pregnancy. It is not possible to identify the cause from timing alone.

Notice the colour and location, but do not insert anything to check. Bleeding with pelvic pain, one-sided pain, shoulder-tip pain, dizziness, or fainting needs prompt assessment because some pregnancy complications can begin with bleeding. A bleeding-between-periods guide is less specific to pregnancy but may help explain the symptom description.

Use a pad or liner to monitor the amount and record when it occurs. Contact your maternity service or clinician for advice, particularly if the bleeding repeats. Mention constipation, straining, pain, discharge, pregnancy dates, and any known blood group information if you have it.

For constipation, ask your pregnancy clinician about fluids, fibre, movement, and suitable treatments rather than starting a medicine without advice. Avoid sex or strenuous activity if it brings on bleeding until you have received guidance.

Seek emergency help for heavy bleeding, severe or one-sided abdominal pain, shoulder-tip pain, fainting, marked dizziness, fever, or feeling very unwell. Contact a clinician promptly for any ongoing or recurrent bleeding in pregnancy, even when the amount is small.
