Spotting 3 years after menopause: what to know

After menopause, vaginal and cervical tissues may become thinner and drier, making light bleeding possible after sex or an examination. Polyps, infection, medicines, or a pelvic condition can also contribute. Bleeding may come from the vagina, cervix, womb, or urinary or bowel tract, so its source is not always obvious.

Most causes are not cancer, but bleeding after menopause can be a presentation of womb or cervical cancer. This symptom does not diagnose either condition; an examination is needed to identify the source and cause.

Contact your GP, gynaecologist, or menopause clinician promptly, even if you have had only a small smear of blood. Record the date, colour, amount, triggers, pain, discharge, and any medicines such as hormone therapy or blood thinners.

Evaluation may include an examination, cervical screening review, ultrasound, or a sample from the womb, depending on your history. Do not assume that bleeding is simply vaginal dryness or restart hormone treatment without medical advice.

Seek urgent care if bleeding becomes heavy, you feel faint or very weak, have severe pelvic pain, fever, or pass clots. Prompt assessment is also needed when bleeding keeps returning or occurs with unexplained weight change, unusual discharge, or worsening pain.

Ask: Where might the blood be coming from? Could dryness, a polyp, medicine, or prolapse contribute? Do I need an ultrasound or womb sample? Are my hormone treatments and screening records relevant?
