Sperm freezing for IVF: what to know

Sperm freezing is also known as sperm cryopreservation. A sample is usually produced by masturbation in a private clinic room, although the fertility team can discuss alternatives when collection is difficult. The laboratory checks the sample, mixes it with a protective solution and divides it into labelled storage containers before freezing.

The stored sample may be helpful if the sperm provider cannot attend on the day of egg collection, if a future medical treatment could affect sperm production, or if a clinic wants a sample available before treatment begins. Freezing does not create new sperm or correct an underlying fertility problem. Some sperm may not survive freezing and thawing, so the laboratory discusses the sample's quality and the treatment plan with you.

Follow the clinic's instructions about abstinence, medicines, identification and sample collection. Tell the team about fever, recent illness, prescribed medicines and any difficulty producing a sample. The clinic may request more than one sample when it needs additional stored material or when the first sample contains too few usable sperm.

Ask how the sample will be stored, how long the consent remains valid and what happens if you change clinics or treatment plans. During IVF, the laboratory thaws the sample and prepares the surviving sperm for fertilisation. Depending on sperm and egg findings, the team may use conventional IVF or inject a single sperm into an egg.

Contact your fertility clinic promptly if you cannot provide the sample as planned, the container is damaged or unlabelled, or you have questions about consent or storage. Seek medical advice for blood in semen, significant testicular pain or swelling, fever, or a new lump. These concerns need assessment whether or not you are preparing for IVF.
