Spectral flow cytometry: what to know

Cells are prepared as a fluid suspension and exposed to light in the instrument. Antibodies carrying fluorescent labels bind selected markers on or inside cells. The machine records the full pattern of emitted light from individual cells, allowing the laboratory to compare marker combinations across many cells and separate mixed populations.

This can help characterise blood cancers, immune cells, or abnormal populations, but it is not a complete diagnosis by itself. Interpretation depends on the markers chosen, the number and quality of cells, the sample source, and the clinical question. Results are combined with the microscope findings, blood tests, genetics, imaging, and symptoms.

Ask which sample was tested and what question the laboratory was answering. Blood, fluid, or a bone marrow biopsy may provide cells for analysis. If the sample contains too few living cells or has been delayed in transport, the report may be limited or another sample may be discussed.

Ask the haematology team to explain the named cell population, the markers that define it, and whether the result changes classification or treatment. Keep the report with other pathology and imaging records. Do not interpret an isolated marker without the full laboratory comment.

The test itself does not usually create an emergency. Seek urgent help for fever with severe weakness during cancer treatment, uncontrolled bleeding, sudden breathlessness, chest pain, confusion, or collapse. After a bone marrow procedure, contact the team for persistent bleeding, increasing redness or swelling, drainage, fever, or pain that is getting worse.

What cell population did the test identify? Which markers support that interpretation? Was the sample adequate? Do I need a bone marrow biopsy or another test? How does this result affect diagnosis, treatment choices, or monitoring?
