Sore throat and headache without fever

Viruses such as the common cold can irritate the throat and cause head pressure or headache before other symptoms appear. Allergic rhinitis may cause a blocked or runny nose, mouth breathing, and a scratchy throat; sinus pressure can add to the headache. Acid reflux can irritate the throat, especially after meals or when lying down, without causing a fever. Strep throat, tonsillitis, influenza, COVID-19, and glandular fever can also involve throat pain, although symptoms vary and fever is not required for every illness.

Drink regularly, choose soft foods, and use warm or cool fluids according to what feels soothing. Rest your voice and avoid smoke or other throat irritants. Saline nasal rinsing may help when congestion or allergy symptoms are present. Check the label before taking any pain medicine, and ask a pharmacist if you have a long-term condition, take other medicines, or are unsure which option is suitable. Do not start antibiotics without an assessment, because they do not treat viral causes.

Arrange a medical review if the symptoms are worsening, persist beyond several days, keep returning, or make drinking difficult. Seek urgent help for trouble breathing, inability to swallow saliva, marked swelling in the throat or neck, confusion, a severe or unusual headache, a stiff neck, or signs of dehydration such as very little urine. A clinician can examine your throat and decide whether testing for an infection is appropriate.
