The short answer

A single temperature reading does not rule out illness. Symptoms may appear before a fever, after it settles, or without a raised temperature. How you feel, how quickly symptoms change and whether breathing or swallowing is affected are more useful than the thermometer alone.

What these symptoms may mean

A common cold, influenza or COVID-19 can cause throat irritation, shivering or feeling cold, tiredness and muscle aches. Influenza often begins abruptly, while a common cold may build more gradually. COVID-19 can have a wide range of symptoms, so symptoms alone cannot identify it.

Strep throat and tonsillitis can cause throat pain and difficulty swallowing; swollen neck glands or visible patches may also occur. Glandular fever is another possibility when marked tiredness and swollen glands accompany the throat symptoms. Allergic rhinitis or acid reflux can irritate the throat, although chills and body aches make an infection more worth considering.

These symptoms do not by themselves show which condition you have. A sore throat is a common presentation of several illnesses, and most people with these symptoms do not need emergency treatment unless warning signs develop.

What you can do now

Rest, drink fluids regularly and choose soft foods if swallowing hurts. Warm drinks, cool liquids or lozenges may soothe the throat; avoid smoke and other irritants. Keep a thermometer available and note whether symptoms are improving or worsening.

Stay home from close contact while you feel unwell, especially around people who may become seriously ill. Wash your hands, cover coughs and consider a COVID-19 test if one is available and relevant to your local advice. Do not start antibiotics without a clinician’s assessment, because they do not treat viral infections.

A pharmacist or clinician can help you choose symptom relief that fits your age, pregnancy status, allergies and other medicines. Follow the product label and ask before combining products with the same active ingredient.

When to seek medical care

Get urgent help if you have trouble breathing, cannot swallow saliva, are drooling, develop rapidly increasing swelling in the mouth or neck, become confused, or feel faint. These signs can indicate a problem needing prompt assessment even without fever.

Arrange medical advice if throat pain is severe, symptoms are getting worse, you cannot drink enough, dehydration develops, or the illness is not beginning to improve. Seek advice sooner for a young child, an older adult, pregnancy, or a weakened immune system. A clinician may examine your throat and glands and decide whether testing is needed.
