Sore skin around the anus and perineum: what to know

Sweat, stool, diarrhoea, wiping, shaving, tight clothing, or fragranced products can irritate this delicate skin. A tender lump may be a boil or abscess. Warmth, spreading redness, swelling, and increasing pain can occur with cellulitis. Repeated painful nodules or draining areas in the groin or perineum may fit hidradenitis suppurativa. Burning pain followed by grouped blisters or a one-sided rash can occur with shingles. Anal fissures and haemorrhoids can also cause pain but are not always primarily skin conditions. Examination matters because different causes need different treatment.

Rinse gently with lukewarm water, pat dry, and change damp underwear promptly. Avoid scrubbing, deodorants, antiseptics, fragranced wipes, and shaving until the area heals. Wear loose, breathable clothing and do not squeeze, lance, or repeatedly press a lump. A simple protective barrier may help when moisture is the trigger, but avoid applying medicines inside the anus unless advised. Tell the clinician about drainage, bowel symptoms, sexual exposure, shaving, recurrent lumps, and any new rash because these details guide the examination.

Seek prompt medical care for rapidly spreading redness, marked swelling, pus, a growing lump, severe pain, fever, chills, or difficulty sitting, walking, passing urine, or opening your bowels. Urgent assessment is needed if you feel very unwell or the skin becomes dusky, blistered, or numb. A painful rash with blisters should be reviewed quickly because antiviral treatment may be time-sensitive. Recurrent nodules or draining openings deserve dermatology review, even if each episode settles.
