What should you know about sodium bicarbonate kidney stones?

A clinician needs to consider the stone type, urine findings, kidney function, blood pressure, medicines, and symptoms before recommending it. Do not self-treat a suspected stone with household bicarbonate.

Some stones form more readily when urine contains particular minerals or is too acidic or alkaline. Sodium bicarbonate can increase sodium intake and alter urine chemistry, so its effect is not suitable for every stone. Kidney stones commonly cause flank or abdominal pain and may cause painful urination or difficulty passing urine, but symptoms vary.

Contact a GP for new stone-like pain, blood in the urine, or urinary symptoms. Assessment may include an examination, urine testing, blood testing, and imaging such as an abdominal ultrasound. Drink fluids according to medical advice, particularly if you have kidney or heart problems. Use only a prescribed dose of sodium bicarbonate and tell the clinician about antacids, blood-pressure medicines, and supplements.

Get urgent medical help for severe or worsening pain, fever or chills, repeated vomiting, inability to pass urine, faintness, or feeling seriously unwell. A blocked urinary system with infection can require prompt treatment. Do not wait for bicarbonate to work when these warning signs occur.

Ask whether imaging is needed, what type of stone is suspected, and whether sodium bicarbonate is appropriate for you. Clarify how much fluid to drink, which pain medicines are suitable, how to collect a passed stone, and what follow-up will check for recurrence.
