Skin rash after fever in an infant: what to know

A rash after fever in an infant should be judged by the baby’s appearance, breathing, hydration, and rash features. Some viral illnesses produce a widespread rash as fever improves, but a rash can also follow a medicine or signal a serious infection. Infants need a lower threshold for professional advice.

A pink, widespread rash after a fading fever may occur during recovery from a viral illness. Hives are raised and itchy, and Drug Rash can begin after a new medicine. Heat Rash is more likely where warmth and moisture collect. The rash alone cannot identify the cause; alertness, feeding, wet nappies, and breathing provide essential context.

Keep the infant comfortably cool, offer usual feeds, and monitor wet nappies and behaviour. Do not apply adult creams, essential oils, or leftover medicines. Note the fever’s course, medicines given, and when the rash appeared; take a photograph for the clinician. Contact the child’s doctor for advice, especially if the infant is very young or seems unlike usual.

Seek emergency help if the infant has breathing difficulty, facial swelling, a seizure, unusual sleepiness, poor responsiveness, or a purple rash that does not fade when pressed. Obtain urgent assessment for poor feeding, repeated vomiting, markedly fewer wet nappies, a stiff neck, blisters, or rapidly spreading redness. Do not wait for the rash to become extensive.
