Signs You’re in Labour Without Your Water Breaking

Labour contractions usually become more organised, stronger, and closer together rather than easing with rest, hydration, or a change of position. You may also notice menstrual-like cramps, pressure low in the pelvis, an urge to open your bowels, or aching across the lower back. A mucus plug or blood-streaked discharge can appear as the cervix softens and opens. The waters may break during labour, or a clinician may break them later; they do not need to break first.

Time contractions from the start of one to the start of the next and record how long each lasts. Rest, sip fluids, eat a light snack if your maternity team has not advised otherwise, and keep your phone and transport plan ready. Call your maternity unit for individual instructions, especially if you are unsure whether fluid is leaking or you have had a previous rapid labour.

Contact your maternity unit promptly for regular painful contractions, vaginal bleeding, reduced baby movements, severe constant abdominal pain, fever, or suspected fluid leakage. Seek urgent help if you are before 37 weeks and develop contractions, pelvic pressure, backache, or a change in discharge, because these can occur with preterm labour. If you cannot reach your team or feel seriously unwell, use emergency services.
