Signs You Need a Tetanus Shot

You may need a tetanus shot after a dirty, deep, or puncture wound, especially when your vaccination history is incomplete or unknown. Tetanus protection is assessed from the type of injury and your previous immunisations; the wound itself does not need to show signs of tetanus before you seek advice.

A wound is more concerning when it is caused by a nail, thorn, bite, burn, crush injury, or an object contaminated with soil, saliva, or faeces. A small puncture can still matter because bacteria may be carried deep into tissue. Tetanus is caused by a toxin made by bacteria entering a wound, not by rust itself. A clean, minor cut may be assessed differently, but vaccination history still matters.

Tetanus symptoms can appear after an injury and include tightening of the jaw, difficulty swallowing, painful muscle stiffness or spasms, and trouble breathing. These are symptoms of a medical emergency, not routine signs that simply confirm a need for a booster.

Wash your hands, rinse the wound with clean running water, and remove visible dirt only if it comes away easily. Apply gentle pressure for bleeding and cover the area with a clean dressing. Do not pour harsh chemicals into the wound or close a deep, dirty, or puncture wound yourself.

Arrange medical advice promptly if the wound is deep, contaminated, caused by a bite or puncture, or difficult to clean. Tell the clinician when the injury happened, how it occurred, and when you last received tetanus-containing vaccines. A clinician may recommend a vaccine, wound treatment, or additional protection based on those details.

Seek urgent medical care for a wound with uncontrolled bleeding, spreading redness, increasing swelling, pus, severe pain, loss of movement, numbness, or a retained object. Go to an emergency department immediately if you develop jaw stiffness, trouble swallowing, repeated muscle spasms, or breathing difficulty after an injury.

If you cannot remember your vaccination history, do not wait for symptoms before asking for advice. Bring any available immunisation record, but lack of documentation should not stop you from being assessed.
