Signs of osteomyelitis on an X-ray

An X-ray shows the shape and density of bone. Infection may produce irregular areas where bone has been damaged, a thickened or altered outer layer, or new bone laid down around an affected area. Swelling in nearby soft tissue can also be reported. These findings are not specific to infection and can overlap with healing, injury, surgery, or other bone conditions.

Early osteomyelitis may not be visible on an X-ray. A clinician therefore compares the image with pain, wound drainage, fever, examination findings, blood tests, and previous images. Additional imaging or a sample from the affected area may be needed when the diagnosis remains uncertain.

Discuss the report with the clinician who requested it, particularly if you have persistent bone pain, a non-healing wound, or drainage near the area. Do not treat an imaging phrase in isolation or use leftover antibiotics. Seek urgent care for rapidly worsening pain, spreading redness, new drainage, fever with chills, confusion, or severe weakness. Urgent review is also needed if the affected limb becomes pale, cold, numb, or difficult to move.

Ask which X-ray findings are present, whether they could have another explanation, and whether the images show early or established change. Ask whether blood tests, advanced imaging, or a tissue sample is needed, and how you will know if treatment is working.
