Signs of Hives: What the Rash Can Look and Feel Like

The patches may be pale, pink, or red and can occur on the face, trunk, arms, legs, or other areas. Individual welts often have a smooth surface and may join into larger patches. Hives can be triggered by an allergy, an infection, heat, pressure, stress, or a medicine, although a clear trigger is not always found.

Itching is often the main sensation, but the skin may also sting, burn, or feel warm. The rash can move or change within a short time, which helps distinguish hives from a fixed patch of dermatitis. Pressing a welt may make it look lighter temporarily. Swelling deeper in the skin, especially around the lips, eyelids, hands, feet, or throat, is called angioedema and can occur with hives.

Hives are a common presentation of urticaria. A rash that stays in the same place, forms blisters, becomes painful, leaves bruising, or comes with fever needs a clinician’s assessment because it may have another cause.

Keep the skin cool with a cool compress and wear loose, soft clothing. Avoid rubbing or scratching, hot showers, tight bands, and any recently introduced product or medicine unless a prescriber tells you to continue it. Note what happened before the rash, including food, illness, exercise, heat, pressure, or a new drug; this record can help identify a pattern.

A pharmacist or clinician may recommend an oral antihistamine for itch. Do not combine products or use a sedating medicine when driving without checking the label or receiving professional advice. Persistent or recurring hives may need medical review to guide treatment and look for relevant triggers.

Call emergency services immediately if hives occur with swelling of the tongue or throat, trouble breathing, wheezing, difficulty swallowing, faintness, confusion, or sudden weakness. These can signal a severe allergic reaction, especially when symptoms develop quickly.

Seek prompt medical advice if the rash is widespread, repeatedly returns, follows a new medicine, or comes with significant facial swelling. Arrange a routine appointment when hives continue, disrupt sleep, or cannot be linked to a clear short-lived trigger. A clinician can decide whether further treatment, such as a corticosteroid course, is appropriate.
