Signs and symptoms of gallstones

A gallstone can block the flow of bile from the gallbladder, producing steady pain in the upper right or central abdomen. The discomfort may follow a meal and can spread towards the back or right shoulder. People with gallstones may also notice nausea or vomiting; indigestion and bloating can have many other causes. Symptoms alone cannot confirm a gallstone, so an assessment may include an examination and imaging.

A gallstone that remains stuck can irritate the gallbladder or nearby organs. This can lead to worsening pain, fever, jaundice, or inflammation of the pancreas. Obesity is one factor associated with gallstone formation, but gallstones can occur without it.

Arrange a GP consultation if repeated upper abdominal discomfort, nausea, or vomiting is affecting meals or daily activities. Note where the pain starts, how long it lasts, whether it follows food, and whether you have fever, yellowing of the skin, or dark urine. Until assessed, choose fluids and lighter meals if these are easier to tolerate, and avoid taking unfamiliar medicines for pain without advice.

Seek urgent medical help for severe or persistent abdominal pain, repeated vomiting, fever with abdominal pain, yellow skin or eyes, confusion, fainting, or pain that spreads to the back. These features can signal a complication such as inflammation of the gallbladder or pancreatitis and should not be monitored at home.
