Pain From the Shoulder to the Thumb: What It May Mean

Pain that travels down the arm with tingling, pins and needles, altered sensation, or weakness can reflect irritation of nerves in the neck or upper limb. Pain centred at the base of the thumb, especially with gripping, lifting, or wrist movement, may be a local tendon problem. De Quervain's tenosynovitis commonly presents with pain on the thumb side of the wrist and difficulty using the thumb. More than one area can hurt at the same time, so the route of pain and the movements that trigger it are useful clues.

Reduce repeated gripping, forceful pinching, and heavy lifting for now. Keep the neck, shoulder, wrist, and thumb moving gently without reproducing sharp or electric pain. A neutral wrist position and a temporary thumb support may reduce strain; use a support only if it improves comfort and does not cause numbness. Note whether symptoms change with neck position, shoulder movement, or thumb use. A clinician can examine both the neck and the arm rather than treating the painful spot alone.

Seek urgent care for sudden or worsening arm weakness, loss of hand coordination, severe numbness, a cold or discoloured hand, or symptoms after a major injury. Get prompt help for chest pressure, breathlessness, sweating, or pain spreading into the jaw or arm. Arrange a routine assessment when symptoms persist, recur, or interfere with sleep or daily tasks.
