Shoulder pain with swelling of the hand

Fluid can collect in the hand when the arm is kept still or held down, but swelling can also involve veins, lymph drainage, inflammation, or an injury farther along the arm. Nerve irritation from the neck may cause pain, tingling, or weakness, but it does not account for every cause of swelling. Shoulder conditions such as rotator cuff tendinitis or frozen shoulder can lead to guarded movement, which may contribute to hand stiffness, yet new swelling still deserves careful review.

Compare both hands for size, temperature, colour, grip, and sensation. Note whether rings feel tight, whether the swelling improves after elevation, and whether there was a recent injection, operation, cast, fall, or long period of immobility.

Remove tight rings or bracelets before swelling increases. Rest the arm in a supported, comfortable position and elevate the hand when practical. Move the fingers gently if movement is comfortable, but do not massage a painful or rapidly swollen limb. Avoid heat over a newly swollen area. Contact a healthcare professional promptly for same-day guidance, particularly if the change is new or unexplained.

Seek emergency help for sudden swelling with breathlessness, chest pain, faintness, or coughing blood. Go urgently if the hand becomes blue, pale, cold, severely painful, numb, or difficult to move. Fever with spreading redness, a hot shoulder, or an open wound also needs prompt medical attention. After an injury, urgent assessment is needed when the arm is deformed or circulation and sensation change.
