Shoulder pain when raising your arm above shoulder height

Short answer
Pain when raising your arm above shoulder height can result from irritated tendons, reduced space around the shoulder, joint stiffness, or a neck-related problem.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Trigger

- Painful arc, stiffness, weakness, neck symptoms, and injury
- Early approach
- Modify overhead work and keep gentle comfortable movement
Possible explanations
Overhead movement asks the rotator cuff and shoulder blade muscles to guide the upper arm while the joint moves through a narrow area. Shoulder impingement or rotator cuff tendinitis may hurt during lifting and lowering. Calcific tendinitis can produce a painful flare. Frozen shoulder usually combines pain with progressive restriction, while osteoarthritis may cause aching and grinding with reduced movement. A rotator cuff tear may present with pain and weakness, particularly after a sudden injury. Cervical radiculopathy can add neck pain, pins and needles, or symptoms below the shoulder.
Compare lifting the arm forward with lifting it out to the side. Also note pain at rest, night discomfort, loss of power, swelling, or a recent change in activity such as painting or gym work.
What you can do now
Temporarily adjust overhead tasks: keep frequently used items within easy reach and take breaks from repetitive lifting. Keep the shoulder moving gently without forcing the painful arc. Support the arm when resting if that is more comfortable. A wrapped cold pack may help after activity; use medicines only as directed and seek pharmacist advice when unsure. Book an assessment when pain is not settling, restricts daily tasks, or repeatedly returns.
Warning signs
Seek urgent care if pain began with a fall or heavy pull and you cannot lift the arm, or if the shoulder is visibly out of place. Immediate help is warranted for chest discomfort, breathing difficulty, faintness, or sweating alongside shoulder pain. Seek prompt review for fever with a hot swollen shoulder, rapidly increasing swelling, or new arm numbness or weakness.

Questions people ask
Why can lowering the arm hurt as well as lifting it?
The shoulder muscles continue controlling the arm while it comes down, so irritated tendons or weak movement control can hurt during lowering too.
Is overhead shoulder pain always a tendon problem?
No. Joint stiffness, osteoarthritis, calcific tendinitis, and cervical radiculopathy can create similar movement pain, so examination matters.
