Shoulder pain when lifting the arm to the side may involve the rotator cuff, joint or neck, so the pattern and strength should be checked. Compare the painful side with the other side without forcing either arm. Pain that changes with position is different from sudden weakness after an injury.

Possible explanations

The shoulder relies on coordinated movement between the upper arm, shoulder blade and collarbone. Rotator Cuff Tendinitis and Shoulder Impingement can present with pain as the arm passes through part of the lift. Shoulder Osteoarthritis can present with aching, grinding and gradually reduced motion. Frozen Shoulder can present with a broad loss of movement, including reaching outward. A Rotator Cuff Tear can present with weakness, particularly after a fall. Cervical Radiculopathy can present with neck pain, pins and needles or symptoms extending down the arm.

Managing the movement

Temporarily bring frequently used objects closer to waist or chest level. Keep using the arm for easy tasks, but avoid heavy or repeated side lifting that reproduces pain. Gentle shoulder-blade movements and comfortable range-of-motion work can be useful when they do not worsen symptoms later. A clinician can distinguish restricted motion from loss of power and guide strengthening.

When an assessment matters

Arrange an appointment if symptoms are persistent, worsening, or affecting sleep, dressing or work. Seek urgent assessment after trauma with sudden weakness, marked swelling or inability to move the arm. Call emergency services for shoulder pain accompanied by chest pressure, breathlessness, faintness or sweating.
