Why a shoulder may click
A click without pain or loss of function can occur as tendons move or joint surfaces shift. When clicking is painful, repeated overhead activity may irritate the rotator cuff or cause shoulder impingement. Deep aching with grinding and stiffness can occur with shoulder osteoarthritis. Sudden clicking after a fall, accompanied by weakness or a feeling that the shoulder slips, may indicate a structural injury. Calcific tendinitis can produce pain and restricted movement, while neck-related nerve irritation can send pain into the shoulder. A sound alone cannot identify the cause.
What to do
Note which movement creates the sound and whether pain, weakness, or catching occurs at the same time. Temporarily reduce repeated overhead lifting and use gentle, comfortable motion. Do not repeatedly force the shoulder to click or push through catching. A clinician can compare both shoulders, test strength and movement, and decide whether imaging or physiotherapy is appropriate. When to seek care
Seek urgent assessment after an injury if the shoulder looks out of place, the arm cannot move, or the hand becomes numb, cold, or pale. Prompt medical advice is appropriate for a hot swollen joint, fever, rapidly increasing pain, or new major weakness. Arrange a routine examination when clicking is painful, persistent, associated with locking, or interfering with work, sleep, or dressing.
