Understanding the location

Pain at the front or inner edge of the lower leg may involve muscles, their attachment points, or the covering of the shin bone. A recent increase in walking, running, jumping, hills or hard surfaces can overload these tissues. Tight calves, worn footwear and a change in training can also alter the load.

Pain that is pinpoint, worsens with impact, continues at rest, or becomes more noticeable at night deserves medical review. A stress fracture is a small area of bone damage caused by repeated loading; it cannot be confirmed from symptoms alone.

Reduce impact activities while the pain is active and choose comfortable, low-load movement if it does not aggravate symptoms. Ice wrapped in cloth may soothe the area for short periods. Wear supportive shoes and avoid testing the pain with repeated jumps or sprints. When to arrange assessment

Arrange prompt assessment for focal bony tenderness, pain that is worsening despite reduced activity, swelling, or difficulty walking. Seek urgent care after a significant injury, or if the lower leg becomes severely swollen, numb, pale or unusually cold. Do not return to impact exercise until a suspected bone injury has been assessed.
