What should you know about sex while spotting after an IUD?

Light spotting can occur after an IUD is fitted as the uterus adjusts. Sex may make blood more visible, and cramping can make penetration uncomfortable. An IUD does not protect against sexually transmitted infections; an infection such as Chlamydia can cause bleeding, pelvic pain, or unusual discharge. Rarely, pain or bleeding may signal that the device has shifted or that another pelvic problem needs assessment.

Use the aftercare instructions from the clinician who fitted your IUD, because advice can depend on the device and the procedure. Check whether you can feel the threads as instructed, but do not pull them. A change in thread length or inability to feel them warrants advice rather than repeated checking.

Wait if sex feels uncomfortable, then resume gradually when you feel ready and your fitting instructions allow it. Use a condom if infection protection is needed, and lubricant if friction adds discomfort. Record bleeding, cramps, discharge, fever, and thread changes. Arrange a review if spotting does not settle, returns after intercourse, or is affecting daily life.

Seek urgent care for severe or worsening pelvic pain, faintness, fever, heavy bleeding, vomiting, or a positive pregnancy test with pain or bleeding. Contact the fitting clinic promptly if the threads feel different, the device seems to be coming out, or sex repeatedly causes pain or bleeding. These signs need assessment; they do not prove that the IUD has moved.
